Hello sir here I am looking for Eligibility form of Mumbai University to get Provisional Eligibility Certificate, so can you tell me from where I canget it and what its fee , also tell me about require document which I have to attached with form

You can get Eligibility form from Eligibility Unit, Room No. 108, Institute of Distance and Open Learning, (IDOL) Dr. Shankar Dayal Sharma Bhavan, Vidyanagari, Santacruz (E), Mumbai-98.

Students will filled up the appication form duly signed by him / her and attached the required documents in Original and self attested Xerox copies.

Documents Required
1 Passport size Photograph
2 H.S.C. and onwards all examination (F.Y., S.Y. & T.Y.) Original + 2 attested xerox copies of marksheet.
3 Proof of Date of Birth. (Any one ) :-Birth Certificate, 10th passing certificate, School College Leaving Certificate
4 Proof of Residence (Any one ) : - Produce residential proof any one Election Card / Driving License / Telephone Bill /Electricity Bill / Ration Card / Society Letter / Agreement
5 Migration / T.C. Certificate.
6 Passing/Convocation / Degree Certificate.
7 In Case of Name Change : Marriage Registration Certificate / Gazzatte Notification (For female married students).
