First of all go to the official website of the Polytechnic University of the Phillipines.
It will appear like this-



Now go to mid bottom of the page, there you will see PUPiApply, click on that and you will get the page as shown in snapshot below-



here you can select your options.

Here I am providing the procedure that how you will get review of your application form after applying for PUP Entrance Test:

So firstly you have to visit the homepage of the official website of Polytechnic University of the Philippines.

On the homepage, you will see a link named ‘Entrance Tests’ in the left side.

When you will click on the link, you will reach on a new page and the page will look like the screen shot:



On this page, you will see a link named ‘After applying online, you can view and print your payment voucher’.

When you will click on the link, you will reach on a new page and the page will look like the screen shot:



On this page, you have to fill following details:
First Name
Middle Name
Last Name
Birth Date

After filling, click on Display Button.
