VTU affidavit Format

Hi Team,

I am a 2016 pass out student from Btu university. I have lost my 7th sem backlog certificate and backlog clearing certificate. So I wanted to request duplicate of 7th sem certificate.

Can you help me with the format? and also when I request for 7th sem duplicate certificate would it come like consolidated of both where backlog is there and cleared or would it come as separate certificates.
