You should have passed Graduate degree in relevant field from any recognized university.
You can apply for Maharashtra Public Service Commission Dental Surgeon Entrance Examination 2012.
Application form can be download from the MPSC official website.

Application form accordingly filled accompany through self-attested Xerox copies of the applicable certificates/ testimonials, along with a cross DD of recommended fee send to required address.

MPSC exam for dental students:

Application Form:
The candidates can purchase the form from the post office (in Maharashtra only) after a payment of Rs100 or can be downloaded from the website. The application forms can be submitted in the mentioned post offices only.

Fee: Rs250 (GEN) and Rs125 (other classes).

The MPSC Notification for dental has not published yet. So When I get it I will provide it to you for further reference.
