Please provide me application form for CCS Exam AHIMA

AHIMA certification distinguishes an individual as competent, knowledgeable, and committed to AHIMA's mission of quality healthcare through quality information.

AHIMA offers computer based certification exams for the RHIA, RHIT, CCA, CCS, CCS-P, CHDA, CHPS and CDIP credentials.

Eligibility:
You must meet one of the following eligibility requirements:

By Credential: RHIA, RHIT, or CCS/CCS-P; OR

By Experience: Minimum of two (2) years of related coding experience directly applying codes; OR

2013 Certified Coding Specialist (CCS) exam will launch on June 1, 2013
