S.S.C pass certificate or equivalent examination certificate showing the Date of Birth
Mark Sheet in Intermediate or Equivalent Examination
Mark Sheet of B.Sc (Nursing) / Post Basic Nursing from 1st to Final year
Degree of B.Sc (Nursing) / Post Basic Nursing from 1st to Final year
Study certificates from 6th Class to Intermediate (10th) and Under Graduate course
Permanent Certificate of Caste (Integrated Community Certificate) for claiming reservation under BC/SC/ST Categories issued through E-Seva or MEE. Seva Officer prescribed
Experience Certificate
Service Certificate (for Service Candidates)
Probation declaration order (for Service Candidates)
Appointment order (for Service Candidates)
Latest Income certificate of the Parent issued by MRO or Tahsildar (for Candidates Claiming Fee Exemption)
Physically Handicapped Certificate (if applied under the quota)

Demand Draft for 1,180/- Rs drawn in favour of “Registrar, Dr. NTR UHS, Vja.” towards verification fee to those who completed B.Sc (Nursing)/Post B.Sc Nursing from out of state.
