Know your customer (KYC) is the process of a business verifying the identity of its clients. Customers of the state bank of India also have to fill KYC form .

Document which the customers have to attach :

Prescribed Documents :

For individual Investors Proof of Identity:
Photo PAN Card

In case of Non Photo PAN Card in addition to copy of PAN Card any one of the following :

Driving License /
Passport copy /
Voter ID /Bank Photo Pass Book.

Proof of Address (any one of the following):

Latest Telephone Bill:
Landline/Mobile

Driving License.

Ration Card.

Rent Agreement.

Latest Electricity Bill

Latest Demat Account statement

Passport copy

Latest Bank Passbook/
Bank Account Statement.

Voter ID.


For HUF

Proof of Identity
any one of the following:

Copy of PAN Card of the HUF.

Proof of address (HUF):

Latest Bank Passbook

Bank account statement

Non individuals (PAN Mandatory) Companies / Bodies Corporate (Certified copy of the following):

Certificate of incorporation.

Power of Attorney granted to its managers, officers or employees .

Memorandum & Articles of Association.

Resolution of the Board of Directors authorizing investment in mutual funds.


Partnership firms (Certified copy of the following):

Any other officially valid documents in respect of holding a power of attorney to transact

Certificates of Registration, in case of registered Partnership Firms.

Trusts, foundations, NGO's Charitable Bodies, Clubs/Mutual Fund Schemes (Certified copy of the following):

Offer Document of the Mutual Fund Scheme

Certificate of Registration, in case of registered Trusts.

Any other valid documents in respect of holding a power of attorney to transact .

• At this page, click on the “forms” option.
• Then click on the KYC form option.
