For non-Filipino citizens, photocopy of Alien Certificate of Registration (ACR).

For those with dual citizenship, photocopy of Philippine Passport or Certificate of Recognition as a Filipino Citizen.

Current school year’s report card with the following grade requirements in any grading period • Average of at least 80% • No failing grade • A conduct or deportment of at least B or its numerical equivalent 4.
