Please tell me that from where can I get application for Central Government Health Scheme Card? Which type of disability candidates can apply for card.

The “Central Government Health Scheme” (CGHS) provides complete health care facilities for the Central Govt. employees their wards that are residing in cities covered by the CGHS.

This scheme was started in year 1954 by the central government.

Following cities are covered under this scheme-

Allahabad ,Ahmedabad ,Bangalore , Bhubaneshwar ,Bhopal ,Chandigarh , Chennai ,Delhi , Dehradun ,Guwahati , Hyderabad , Jaipur , Jabalpur , Kanpur , Kolkata , Lucknow , Meerut , Mumbai , Nagpur , Patna , Pune , Ranchi , Shillong , Trivandrum and Jammu.
