IRDA Form 12

Hello sir, is there any one can give me here application form of IRDA for Annual Submission of Return to the Authority 12?

In IRDA you need to submit the Form -12 to IRDA. The Form 12 includes information related to the number of surveys conducted and additional qualifications obtained (if any) during the financial year.

It is mandatory to submit Form 12 for every financial year.
