The Application Form of CDS 2016 can be filled online.
It will be filled in two phases

Applicants can find the form from the official website of Union Public Service Commission (UPSC) website.
Search for it and register for it.

The Application form will be loaded on the screen. Enter the information asked like-

i. Name of the applicant
ii. Gender
iii. Category
iv. Father’s name
v. Mother’s name
vi. Nationality
vii. Marks in %
viii. Qualification degree
ix. Board/ University
x. Contact info etc

This is the part 1 of CDS 2016 Application Form.
Now, it’s the time to pay the registration fee which is Rs 200 for general category candidates.
After paying the fee, proceed towards the part 2 of the application form.
Upload the photo and signature and finish the part 2.
Submit it for completing the procedure.

Mode of Payment of fee: Fee can be paid either via:
Online mode: Credit/ Debit card.
Offline mode: Submitting the e- challan at the SBI bank only
