Candidates can fill AMU Medical 2016 application form either online or it can be downloaded

The downloaded application form of AMU MBBS BDS 2016 should be filled neatly and no overwriting or scratching should be done

Candidates must fill all the relevant fields given in the AMU Medical application form 2016

Candidates who will fill application form online have to take a print out of the duly filled in AMU MBBS BDS Application Form 2016 on A4 Size paper

Candidates have to offer their signature, thumb impression and your fresh passport size photograph at the designated place on the filled in application form

The cost of examination fee will be Rs. 1000 which can be paid either through online payment gateway or through bank challan or demand draft in favour of ‘Finance Officer, AMU, payable at Aligarh’

Physically Challenged/Blind candidates need not deposit any Application Fee and may write ‘0000’ in DD column

Candidates who use Online approach for payment and do not get their User ID / Password for downloading the PDF Copy of the duly filled in Form, are requested to contact the
Admission Section, via helpdesk numbers or in person and tell their name, father name, course applied for and date of payment and wait for 24-48 hrs.

If their payments are victorious, the details for downloading the PDF copy of the form shall be emailed to them on the email IDs provided in the Application Form

The filled application form along with requisite documents have to be submitted either personally or through registered post/speed post/courier

The application form must reach the Controller of Examination before the last date of receipt of applications

Supporting Documents to be attached with AMU MBBS BDS 2016 Application Form:--

Candidates need to attach the photo-copies of the following documents along with duly filled in AMU MBBS 2016 Application Form

Self-attested copy (both sides) of High School/Secondary School Certificate in support of Date of Birth
Self-attested copy (both sides) of Mark sheets of qualifying examination
Self-attested copy (both sides) of Certificates claiming reservation (if applicable)
Other relevant documents

Note: The candidates however, need to provide the original Demand Draft/Bank Challan Copy of the stipulated fee and put his/her name, Date of Birth, Course of Study and Transaction Id on the reverse of the Demand Draft/Bank Challan Copy
