Application form for OD

Hi,

This is Himabindu.M,I completed my P.G (M.C.A) in V.S.R.I.C.E,ELURU in 2008 which is affiliated to Andhra University Vizag.Now i want to get my Original Document of my M.C.A Certificate.Let me Know the process and how to download the Application form to apply for OD.And Let me know in how many days i can get my OD.
