Entrance CS

I want to give MS in CS entrance exam so I want to know about eligibility and please tell me what documents should be submitted along with the application form?

Eligibility:
The candidate must have baccalaureate degree in Computer Science or a related field from any recognized university.

Documents Required:
TOEFL scores
Two copies of all official, attested transcripts and/or attested diplomas in both English and the language of original, official documents
Transcripts of all undergraduate and graduate education
Three letters of reference
A letter outlining significant work experience and academic and career objectives
GRE scores
