General lnformation for Nursery Application for the session'- 2019-20
1. Please read the 'General lnformation'carefully before filling in the Application form.
2. All entries should be made in ENGLISH and in BLOCK LETTERS.
3. This Application form is for the Nursery class ONLy.
4. Eligibility: Date of Birth - Born between 1st April 2015 and 31st March 2016 (both days included).
5. Proof of Age: MunicipalCorporation birth certificate. (maxsize:500kb, format: pdf)
For Catholic students Municipal Corporation and Baptism certificate. (max size
500kb, format : pdf)
6. Please attach anv one of the following as proof of your address. No other proof will be acceptable.

a. Aadhaar card
b. Current Electricity Bill
c. Voter's ldentity Card
d. Passport
7. Family photograph (Mother, Father and child) to be uptoaded with the form. (max. size:500kb,
format : jpg, jpeg)
8. Proof of income - salary statements of both parents (max size : 500kb, format : pdf).
9. The cost of the application form is Rs. 200/-.
L0. The application form will stand cancelled in the following cases:

b. Recommendation / reference letters
11. The list of candidates selected for an lnteractive session will be displayed on our website on the
specified date. Please do not make any enquiries before that.
12. Parents of selected candidates must carry the original documents and
