City Colleges transcripts

I want to request for the transcripts from the San Jose City College so give me process for doing request of transcripts ?

Here I am giving you information about the transcripts request to the San Jose City College

The first two official transcripts offered by san Jose City College are free of cost

Transcript fees:
Regular Unofficial (7-10 working days to process): $4 per copy
Regular Official (7-10 working days to process): $4 per copy
Rush Official (24 hours to process): $10 per copy

For requesting the transcript you have to submit an application form with the followings.

Written requests must include the following:
Student’s Name (Include former name if applicable)
Student’s SJCC student ID number OR Social Security number
Student’s date of birth
Student’s contact information including: phone number and e-mail address
Students legal written signature
Method of payment, if applicable (cash, personal check or credit/debit Visa/MC)
Approximate dates of attendance
How many copies are requested
Names and addresses for location(s) of transcript copy(s) to be mailed to
If payment is required you can pay by including your VISA/MC number and expiration date next to your signature
