A complete application includes:
1. A properly accomplished application form of the UPCM Application forms will be made available upon cash payment of an application fee of Php 1,500.00* which is non-refundable.

2. Original/Official Transcript of Records for at least the first three and one-half (3-1/2) years of Baccalaureate Program. For the Intarmed applicants, the Fourth year high school report card for at least.

3. Grading periods must be submitted.

4. Four copies of 2x2 pictures signed by the applicant on the front of the picture.
