Admission checklist

Would you plz tell me that where we need to send Admissions Checklist from for conformation of admission at School of General Studies
Columbia University and tell me also that what document will be attached with our application from?

I think you are first-year students so before you send your Admissions Checklist from you have to attach following document with it:
A complete application including parts I and II.
Autobiographical Essay.
Two letters of recommendation.
$80 application fee made payable to Columbia University.
Photograph (recommended, but not required).
