Gurgaon Form of M.Sc in Math

I want to take admission in M.Sc in Math at ITM University,Gurgaon,so can you provide me the application form for admission?

ITM University has been setup by the Haryana State Legislature under The Haryana Private Universities Act (Third amendment), 2009.
It is a Private Self Financed University and affiliated by UGC,NAAC.The university offers the M.Sc in mathematics.

Eligibility for M.Sc(Mathematics):

The candidates must be BSc / BA(Hons.) in Mathematics with 55% or any other Bachelor degree.

Fees:
Rs. 45,000/- (Per Semester)

Department of Applied Science of the ITM University, Gurgaon offers Master of Science degree in Mathematics.

Duration : Two Year (4 Semesters)

INTAKE:
20 seats.

Eligibility Criteria:

Candidate who has passed B.Sc or B.A Hons. in (Mathematics) or any other Bachelor with a minimum of 55% marks in the aggregate eligible for the course.

Candidate from a university NOT recognized by UGC/AICTE/AIU/MHRD or any other statutory body of Government of India shall not be eligible for admission.


Selection Procedure:
Admission shall be made strictly according to merit based on B.Sc or equivalent marks with marks in Mathematics in graduation .
