If you want to take admission in ISB you have to follows these eligibility criteria:
The candidate must graduate in any discipline.
Two years of full-time work experience after graduation is preferable.
GMAT Scores of the last five years are also valid for the course.

If you are fit in these eligibility criteria you are able to take admission. Appellations form can download from the official website. And you also obtain the form form the administration office.
And you have to give the GMAT exam to take admission.
