As you want to get the registration form of CPAT Examination at the Chippewa Valley Technical College. So here I am providing the registration form.

Minimum Department Requirements

Fire Department Antigo

Age: Minimum age must be 18 years.

The candidates must have a Valid Driver's License.

The candidates should have High School Diploma or GED/HSED.

They should Nationally Registered EMT-Paramedic (or eligible for these credentials).
