Default Japanese Language Proficiency Test

What is the pattern of Japanese Language Proficiency Test and what is the minimum eligibility to apply for this test? Does all the university in Japan required JLPT to clear before giving the admission?

Japanese Language Proficiency Test (JLPT)
The Japanese Language Proficiency Test is meant for foreigners learning the Japanese language to measure and certify their proficiency in the Japanese language. This test is conducted both in Japan and overseas during the months of July and December in Japan as well as in overseas.

(The July Test is conducted only for certain grades, countries and regions.)
Some universities impose this test on students from countries that do not conduct the "Examination for Japanese University Admission for International Students" (EJU) who had applied for an undergraduate, master's or doctorate programs of Japanese Universities.
