Successful completion of an entrance exam

Complete the Application Forms

$50 Application Fee

Driver’s License or State ID

Official High School Transcripts

Passport Type Photo for Student ID (Mail or Email in JPEG Format)

Signed Social Security Card

Copy of High School Diploma or GED

Additional Requirements for Nursing Applicants:

Applicants may also have the following scores: Reading = 85%, Math = 65%, Science = 60% and English = 70%.

Test of Essential Academic Skills, Version-Five (TEAS-V).

Reading = 85%, Math = 65%, Science = 60% and English = 70%.

(HESI) with a score of 70 or higher on (

1) English Language composite Score comprised of reading, grammar, vocabulary,

(2) math composite score.

Clear Level II background check

Proof of ability to meet the Technical Performance Standards

(3) letters of recommendation from persons other than family members (e.g. Employer, Instructor, Pastor, colleague, etc.)
