REPORTS TO: BLENDED CASE MANAGEMENT COORDINATOR

JOB SCOPE: Provides case management services to clients. Works on blended team to provide individualized service plan for meeting the needs and goals of the client.

EDUCATION AND EXPERIENCE:
Applicant must have Minimum two years experience in providing direct services or in a substantial number of activities outlined in Section F of the Case Management Guidelines.

Services must have been provided to people who are mentally disabled or homeless.

Bachelor’s degree in one of the OMH approved fields is required, or the bachelor’s degree may be substituted with one of the following:

One (1) year of case management experience and an associate’s degree in health or human services field, or

One (1) year of case management experience and an additional one year of experience in other activities with the target population, or

A bachelor’s degree which includes a practicum encompassing a substantial number of activities with the target population, or

Individual meets the regulatory requirements for case manager of a State Department within New York State.
